What is Hospice?

When cure is no longer possible, hospice may be an option to provide someone you love a way to have the best quality of life for the time remaining.

Hospice allows someone to live life with comfort and dignity. Hospice can occur at home surrounded by familiar, comfortable settings, and caring family and friends. Hospice can be at a facility such as an assisted living or a skilled nursing facility.

 

We provide care wherever the patient considers home.

When to Consider Hospice?

  • When your loved one requires more help or assistance with daily needs

  • Loss of awareness of surroundings

  • Rigid muscles 

  • Difficulty sitting or holding head up 

  • Inconsistency with verbal communication

  • Weight loss

  • Trouble swallowing

  • Incontinence of bladder or bowels

  • Recurrent infections

  • Pressure ulcer

Make an appointment with your Primary Care Physician to talk about your care needs and request for support.

Hospice Care Eligibility

All patients with a life-limiting or terminal illness qualify for services if:

  • Their primary care physician certifies a prognosis (or life expectancy) of 6 months or less.​

  • They have completed and are no longer receiving curative therapies or treatments.

How do I get Hospice?

  1. Talk to your Primary Physician/Oncologist/Specialist:
    Ask them for an ORDER for Hospice along with the History & Physical (H&P) to be faxed to the hospice of your choice. Our fax number is 408-444-5544.

  2. Make sure to have your insurance provider information:
    Medicare card handy to share the information with our office.

  3. We will contact you to schedule admission after we review the documents

The process can vary with the time frame, it can happen within 24 hours or longer depending on the time it takes to request documents from the physician to the hospice.  

Benefits of Care

We provide:

  • Hospice nurses, social workers, chaplains, home health aides and volunteer visits to the patient at their residence.

  • An on-call hospice nurse, 24-hours a day.

  • Medications for pain and symptom management related to the terminal illness.

  • Durable medical equipment and supplies.

  • Counseling and spiritual support for the patient and family.

  • Respite care for the caregiver. 

  • Anticipatory grief support for all family, friends and loved ones.

Where is the Care Provided?

Care may be provided in the home, a skilled nursing facility or residential care home (assisted living facility).

What is the Cost of the Care?

Our mission is to provide services to all terminally-ill patients, regardless of ability to pay. Care is generally covered under one of the following: Medicare hospice benefit, Medi-Cal hospice benefit, most insurance plans (HMO/PPO/Private) and other managed care organizations.  Hospice services are never billed to the patient or family, regardless of their insurance status.
